If schools using an active consent process choose to use Maro's in-platform parent communication tools, parents will receive a version of this email customized to their school and child. This email is available in English and Spanish.

Your school is checking in on how students are doing. Starting {{ScreeningDate}}, students will answer a short set of questions about their mood, stress, and general wellbeing. The survey takes about 10 minutes and happens during the school day.

As the parent/guardian of {{StudentName}}, you get to decide whether they take part in the survey. Please submit the parental consent form below and choose whether to "Opt In" or "Opt Out."

If we haven't heard from you by {{ConsentDeadline}}, your child will not take part in the survey by default.

Please note that your response covers the full school year. If you wish to opt in or out it must be done through this form. School staff can't do it on your behalf.

What happens afterwards?

After your child completes their survey, a designated staff member at the school reviews the answers. These answers are kept strictly confidential, and survey results will never appear on your child's transcript.

If your child's answers suggest they could use extra support, your school will contact you directly and may offer some optional resources. The survey is not a diagnosis. Schools conduct these surveys in order to notice sooner when a student might be struggling.

What is Maro?

Your school is using Maro, a third-party vendor, to conduct these student surveys. Maro is not a treatment provider and assists only with the administrative side of the survey.

Maro is on a mission to help close the 11 year average gap between the time kids start showing signs that they are struggling and the time they get meaningful support. The team is dedicated to helping families get the right support, at the right time, and access the resources they need to thrive.

Parental Consent Form - Opt In

Hi {{parent_first_name}},

Your child's school is planning to survey students for wellbeing and mental health soon and will be using the Maro platform, a third party vendor to the school, to conduct student surveys. This survey is completely optional. If you want your student to complete the survey, please complete this form. If you do not want your child to complete the survey, no action is needed.

Among other things, the survey tools may help your child’s school counselors and other professionals identify individuals who may have or may be at risk of developing certain mental health conditions including {{insert condition based on screener chosen in screening plan}}. Surveys will start on {{insert date}}. 

Maro will display student survey results to the applicable school personnel for further review. These surveys and the school’s subsequent review do not generally mean that students are receiving diagnoses or mental health care at school. However, school personnel may make recommendations based on his/her review of the survey results.

If you have any questions regarding this survey, please contact your child’s school for additional information. Completion of the survey is completely voluntary. For an overview of the survey, please follow the link here. 

Please note the following:

• Your school has an opt-in consent policy. This means that if you do not submit a consent form, your child will not be considered eligible for screening.

• You MUST sign and submit an opt in form at least 24 hours before the first date of screening in order to have your response recorded.

• You can not reach out to your child's counselor to have them opt your child in or out on your behalf. 

• Opt in applies for the entire school year, unless your school specifies otherwise.

• Opt in forms are uniquely generated for each parent or guardian. Please do not forward another parent your opt in form; your form will ONLY apply to your child who is named on the form itself.
